LCSW Licensure Walk Through - Required Supervised Experience
Required Supervised Experience
You cannot begin accruing supervised experience towards licensure until you are registered as an Associate Clinical Social Worker (ASW) with the Board.

Supervision Forms
The Board has three (3) required forms you will need to become familiar with in regards to supervision and accumulating your hours. The Board also has a weekly tracking resource available to ASWs. This form is optional and is NOT REQUIRED. All of these forms are available in the Forms and Publications section of this website.
- Responsibility Statement Forms for Supervisors of an Associate Clinical Social Worker– Your supervisor should sign this form at the commencement of supervision.
- Supervisory Plan – You should have one plan for each employment setting. This form should be filled out with your supervisor.
- Clinical Social Worker Experience Verification – This form will be used to verify all the hours gained in a particular setting or under a particular supervisor.
- ASW Weekly Tracking Log – This resource allows you to track experience gained on a weekly basis. It is purely a resource for you and your supervisor. Do not send this form in to the Board. This form is not an official documentation of supervised experience.
Important Reminders Relating to Supervised Experience
- Keep track of your supervision paperwork. The Board does not require you submit any forms until you are ready to apply for examination eligibility. This includes all Responsibility Statements, Supervisory Plans, and Clinical Social Work Experience Verification forms. Make sure all these forms are complete and contain original signatures. When the time comes to send them in to the Board, you will need to send in the originals. Always make copies of forms for your own records.
- Be sure to meet with your supervisor for at least one (1) hour of individual supervision or two (2) hours of group supervision every week. In order to count any work experience in a given week, you must meet this minimum requirement.
- Utilize the Board’s Verifications feature to check that your supervisor has a current license. If your supervisor does not renew his or her license, the hours you earned while your supervisor’s license is delinquent will not qualify.
- Take time to review the required hours of experience for licensure so that you know the types of work experience on which you need to focus.
The Six Year Rule
ALL HOURS OF EXPERIENCE MUST BE GAINED WITHIN THE MOST RECENT SIX YEARS FROM THE DATE THE BOARD RECEIVES YOUR APPLICATION FOR EXAMINATION ELIGIBILITY.
Example: Applicant A sent in an application for LCSW examination eligibility, and the Board received the application on 4/3/2006. The Board will only accept the experience gained between 4/3/2000 and 4/3/2006 towards the experience requirement.
